Hysteroscopy: An Incisionless Surgery for Reproductive Issues
Hysteroscopy is a medical procedure in which a hysteroscope (a thin, lighted tube) is inserted into your womb through the vaginal opening and the cervix. The tool gives your doctor a clear view of your uterus and the surrounding organs. If anything seems abnormal, they can also take a tissue sample. It can be diagnostic or operative. Sometimes, the surgeon performs unplanned hysteroscopy surgery to remove fibroids and polyps and treat other medical conditions that would otherwise require an open, traditional surgery.
What sets it apart from other reproductive surgeries is it’s minimally-invasive and incision-less nature. You can expect a quick recovery and a short hospital stay.In fact, most women who undergo diagnostic hysteroscopy can expect to leave the hospital the same day.
Procedure
Before hysteroscopy, the doctor will ask the patient to take a pregnancy test, as the procedure can’t be performed on a pregnant woman.
Here’s how it’s carried out:
- Dr. Kavya uses local or general anesthesia to ensure that you feel no pain or discomfort during the surgery.
- Dilators can be used to open your cervix to get quick access to your womb.
- To keep your vagina open, the doctor uses a speculum and then inserts a hysteroscope through the cervix.
- A carbon dioxide gas or special fluid is passed through the device into the uterus so that the area that has to be operated on is clearly visible.
- Your uterus with the fallopian tubes and the surrounding organs will be visible on the monitor.
- The surgeon will pass other instruments to break the fibroids, polyps, and other growths into pieces and remove them through the hysteroscope.
Since it’s done vaginally, there’s no incision involved.
Types of Hysteroscopic Surgeries
Having performed different types of hysteroscopic surgeries, Dr. Kavya has become a top-rated hysteroscopic surgeon in Mumbai. She works at hospitals that are well-equipped with cutting-edge technology and equipment to perform nearly all types of gynec surgeries. She also specializes in handling emergency cases. Here are the surgeries that can be performed with a hysteroscope:
Myomectomy and Polypectomy
Myomectomy is a surgical procedure to remove fibroids. Myomectomy can be performed with a hysteroscope vaginally if the fibroids are small and submucosal. Dr. Kavya will check your symptoms and the location of the fibroids to determine if hysteroscopic myomectomy will work. It’s carried out with great precision, ensuring that only fibroids are removed, leaving the uterine tissues intact. This increases your chances of getting pregnant and having a viable pregnancy. Similarly, polyps in the uterine wall can be removed with a hysteroscope. The doctor will locate them inside your uterus and use a hysteroscope to remove them carefully.
Adhesiolysis
A history of uterine surgery can cause scar tissue formation in your uterus, which leads to the narrowing of the uterine cavity. The symptoms include pelvic pain, heavy menstrual bleeding, and infertility. Adhesions can be removed with hysteroscopy. However, the surgeon might schedule another hysteroscopy to check if the adhesions are removed properly and the uterine tissues are preserved.
Removal of Foreign Objects
Foreign objects like Copper T can be misplaced due to incorrect insertion, strenuous exercises, or strong contractions. To bring it back into its original position or expel it completely from the uterus through the vagina, hysteroscopy surgery might be needed. Other foreign objects, like retained tampons, placental tissues, and fetal tissue (in case of miscarriage or abortion), can be taken out safely with hysteroscopy.
Endometrium Ablation
Another common procedure that can be performed with a hysteroscope is endometrium ablation — the removal of the endometrium to control symptoms like heavy bleeding and prolonged periods. It’s the endometrium that sheds during your menstrual cycle. So, without it, your period will either stop completely or your bleeding will be very light. Either way, endometrium ablation can significantly improve your symptoms.
Hysteroscopic Septal Removal
A uterine septum is a congenital anomaly that involves a tissue dividing the uterus into two parts, making it difficult for the woman to conceive. The inability to get pregnant or frequent miscarriage can be due to uterine septum. It can be corrected with a surgical removal of the tissue that separates the uterus.
Recovery After Hysteroscopy
Recovery is short and doesn’t require any strict guidelines. Dr. Kavya will give you instructions on how to care for yourself and what to avoid during recovery. Generally, women undergoing hysteroscopy are asked to avoid driving, taking a hot bath, or swimming for 1-2 weeks. You must avoid intercourse, douching, and inserting anything into your vagina until your doctor clears you for these activities. A hospital stay is not needed unless general anesthesia is used. Mild cramps and nausea are normal after hysteroscopy. This can be accompanied by mild vaginal bleeding.
Severe complications of hysteroscopy are rare but possible. Watch out for the signs of infection and report any heavy vaginal bleeding to your doctor immediately.
Hysteroscopy can damage the surrounding organs. To prevent serious complications, it’s important to work with a professional and board-certified gynecologist.
