Understanding Intracytoplasmic Sperm Injection (ICSI)

ICSI is a part of In-vitro Fertilization (IVF). The treatment is recommended when the male partner is infertile. ICSI can help you with conception if you are diagnosed with azoospermia or poor sperm quality.

The procedure involves injecting a live sperm into a fertile egg with a micro-needle to increase the chances of fertilization.

The doctor chooses sperm with the best motility and appropriate shape and fertilizes it with an egg in a petri dish.

The entire procedure is conducted in a laboratory equipped with the tools and equipment to carry out ICSI and IVF efficiently.

ICSI also works for women with low ovarian reserve.

ICSI Procedure

Intracytoplasmic Injection is an effective treatment for partners with male factor infertility, as well as women with low egg count.

Here’s a detailed overview of the ICSI procedure.

Physical Test

Before ICSI in Mumbai at Dr. Shetty’s Clinic, your doctor will schedule a physical examination to evaluate your health and determine whether or not your body is fit for in-vitro fertilization.

They will order bloodwork and ultrasound to get a clear picture of your ovarian reserve. This also gives them a better idea of the hormone dosage required for ovarian stimulation.

Ovarian Stimulation

The doctor will schedule hormone medication that stimulates your ovaries to produce multiple eggs simultaneously. These eggs mature and are released from your ovaries within 8-14 days of the hormone shots.

Egg Retrieval

Once your eggs have developed, the doctor will inject medication to mature these eggs and perform egg retrieval within 36-48 hours.

It’s performed under light sedation on an outpatient basis. The doctor will insert a thin needle into your cervix through the vagina.

Using an ultrasound guide, they will direct the needle to the fallopian tubes, pierce them, and retrieve mature eggs. These eggs are then transferred to a laboratory.

Sperm Collection

Around the same time, your partner must provide a semen sample. It’s collected in a sterile container and sent to the lab for semen analysis.

Your doctor will test the sperm to collect the healthy, potent sperm with good motility. The sperm are further treated with a specialized solution to prepare them for fertilization,

Fertilization

In IVF, sperm and eggs are combined in a petri dish and left overnight for fertilization. However, in the case of male factor infertility, the sperm is manually injected into the oocyte.

It’s done through a thin needle that sucks up the selected sperm and is then used to inject it into the mature egg. This improves the chances of fertilization.

Embryo Culture

Once fertilization occurs, the doctor carefully monitors the growth of the fertilized egg until it reaches the blastocyst stage, which occurs around day 5 or 6 of fertilization.

The embryo is stored in a controlled environment with the same pH and temperature as the mother’s uterus.

Embryo Transfer

The last stage of the procedure is embryo transfer, which takes place 5-6 days after egg retrieval. In an embryo transfer, the chosen embryo is injected into the uterine cavity through the cervical opening.

If required, the doctor will recommend progesterone medication a few days before the embryo transfer. This prepares your uterine lining for a successful implantation.

Pregnancy Test

You are advised to take a home pregnancy test in 10-12 days after embryo transfer. The doctor might also order a blood test to measure the hCG levels in your blood.

If the implantation is successful and your pregnancy test is positive, you will be monitored carefully for the first three months.

What’s the Success Rate of ICSI?

ICSI is a highly effective procedure with a 75-80 percent success rate. That means there’s a very good chance that fertilization will occur if the sperm is manually injected into the mature oocyte. The success rate, however, varies from woman to woman.

The lab setting, infrastructure, and doctor’s expertise might also affect the success of ICSI.
